What are some interesting mass media ethical scandals?

I have to write a paper on mass media ethical scandals. Anything where someone in the media acted unethically and got caught. Any ideas?

Dan Rather from CBS News reported as factual unverified mis-truths (lies) about George W. Bush’s military service. He was forced to resign in disgrace when the real truth was revealed.
